Using the ParTNers Center (or an in-network provider)

To earn your cash incentive under the wellness program, you must download a physician screening form and take the form with you to your screening appointment. First, set up an online account with Quest, the biometric screening vendor, by clicking here. You will need the registration key: SOT. Then follow the instructions to download your physician screening form, and call the ParTNers Center at 615-741-1709 to schedule your screening appointment.
