When you visit the ParTNers Health & Wellness Center, we trust you will experience a welcoming environment with our friendly staff. Here are a few things to know before your first visit:

 

Please bring:

• Your State health insurance ID card – This serves as proof of your enrollment in either the PPO or CDHP. We will not use this card for any other purpose.

• Your State employee ID or driver’s license – You need one of these for identification purposes and to gain access to the building.

Arrive a few minutes early to complete some paperwork

When you arrive, we will ask you to fill out a few patient information forms. This information will be entered into your confidential Electronic Health Record.

 

Learn about the patient portal

Through the Center, we offer a patient portal called FollowMyHealth. This is a secure, password-protected website that allows you to stay in touch with your Center providers after your visit. Through it you can:

• Schedule an appointment

• Access your Electronic Health Record* (including labs and medications)

• View instructions from your ParTNers Center provider after a visit

• Ask a ParTNers Center provider a question

• Request a refill

• View patient education materials

 

At any visit, ask our front desk staff for an invitation to access the patient portal. The invitation will be sent to you via secure email with instructions on how to log on.
